I am suffering from secondary infertility.Have a
10yr old kid.Trying to concieve for 4yrs now.Had hysteroscopy and
laproscopy two yrs ago.Have PCOD.My husband's report is good and my
hormones tests are normal.Have tried 6 IUI and 1IVF-all
failed.Conclusion was that my endometrium lining does not grow beyond
6mm although there are 3 lines.Ovulation happens.Have been taking
metmorfin regularly.Early this year i concieved naturally which was a
surprise but had a missed abortion in 6wks.What are my chances of
getting pregnant?Should i try IUI again?Ultrasound shows lining 6mm at
the time of ovulation with Letroz during this month's cycle.Did i
concieve with thin lining only?Should i go for surrogacy?

Answer:
Sorry to hear about your problems. Since you did conceive naturally, the
lining may have been normal at that time. Were you taking clomid ? That
would make the lining thin. However, you should not face the same
problem with letroz. Sometimes, the lining can be improved by giving
estrogen tablets. Also, since you have had a missed abortion, consult
your gynec. regarding progestational supplementation after ovulation.

Dear Doctor, Please help us by providing your
valuable suggestions. In the month of June,I had written here the same
but unfortunately my question was unanswered. We are married more than
1.5 years and I have not yet conceived. I am 28 and my husband is 34. In
April,we had done my hormonal tests and semen analysis for my husband as
per our gyanc's advise .The sperm count was only 1-3 million out of
which 50% was motile and in that 3 % actively motile and 47% sluggishly
motile. My husband was also diagnosed with a grade 3 veicocele. He had
taken ADDYZOA daily 4 tabs (2-0-2) for one month and took UBIPHENE daily
one tab in the night for 90 days.
My doctor had put me on the following medicines, though my clinical
examination and hormonal results were ok .I have been getting regular
monthly cycles too.
ENEW 400( daily 1) FEMIX (daily 1) -(Morning)
M2 TONE twice a day
OVOFAR -from 2nd day of periods to 6th day
I took the above for the last 1.5 months and took Ovofar for the periods
in May. I was asked to continue with the medicines till I get
conceived.But taking my family doctor's advice I have stopped the
medicines. My husband's semen analysis report after 3 months of the
surgery(now in August) showed the same count of 2 million out of which
20% was only motile and in that rapidly progressive was NIL .Now the
doctor has asked to stop Ubiphene and start with Siphene 50. for 45 days
My questions are
·What are the chances of pregnancy for us with this sperm count?
·Will there be a significant increase in the sperm count?
·What treatment should we exactly go for?
·Could you kindly advise on my medicines and let me know why I was
supposed to take the medicines, as I do not have any complications as of
now? How are these going to help in getting conceived?

Answer: I was on leave and we have many questions, so was unable
to answer your question earlier. Are your husband's FSH and Prolactin
hormones normal? Is his blood sugar normal? Siphene is fine. Check the
semen again after 2 months and if improved , he can continue the same
medication and you can try IUI. If it has not improved, you would
require ICSI . let me know, so I can advice you further. |

married. After coming to UK my periods got irregular and after coming
for holiday to india I ahd a check up and doctor told that I have pcos
and she adviced me to use novelon. I used novelon for 3 months and now
from 2 months I have stopped using it my periods became regular.... and
we r planning for a baby from 2 months.. but no use,but my periods r
very regular ie for 28 days, please can u give me advice......please

Answer: You have to determine whether you are ovulating or not.
You can do an ovulation detection test at home on the urine. This will
also indicate your most fertile period. If you are not ovulating, you
might require medication like clomid. . Consult your gynec. for that.
Also, have your husband's semen examination done and your tubes

Dear Dr. My wedding is on 8 September 2007, and my
period is expected to start on the same date as i have a regular one.
The Pharmacist suggested that i take Primolut Nor starting 20-8 till
24-8, 2 pills a day, so it will come directly after that and will be
done before my wedding. Today is 28 August and my period hasn't arrived
yet. what can i do now ... i really need it to be done before the
wedding. Shall i take the medicine again? or shall i just wait?

should start Primolut N twice a day from one week before the date you do
not wish to get a period till the date you want the period delayed. |

Answer: If you are planning to conceive stop the ginette during
the next cycle. Continue the metformin and folvite. Check whether you
are ovulating or not by doing an ultrasound on day 12 of the cycle . If
you are not ovulating, you might need other medication like clomid. Your
gynec . will be able to advice you on that. |

Answer: Do you have polycystic ovaries or a cyst ? What is the
size of the cyst? Women with polycystic ovaries have a high insulin
level and diamet would help to regularise your periods and help you
ovulate. You may conceive, so you should use some contraceptive. Have
you done a breast ultrasound or mammogram for the breast pain? Also, do
a prolactin hormone test. let me know the results, so I can advice you
further. |

I am 26yr old, married for 1.5 yrs. I have PCOS
since last year.First I was taking bigomet 500 twice a day then started
having side effects of the same like breathing problem, headache,
acidity. so mu gynae suggested to take nacfil and do diet control. Now
im planning for a baby...will b starting my dose of nacfil just wanted
to confirm if it is fine.Also, PCOS at early stages hints at diabetes at
later stages??

Answer: NI have not used Nacfil, so I cannot advice you regarding
that. PCOS is associated with diabetes, so you should ensure that you
have a healthy diet and plenty of exercise always, to reduce the risk of
developing diabetes. Also, check your blood sugar levels every 2-3
months. |

Hi Doctor. I just went for my second IUI on the
21st of August. I was given 150mg clomid. Now its 7 days after i have a
slight brown discharge, little brown spotting and also stomach pains
like I'm about to get my periods. What may be the reason? How many days

Answer: You can do a blood test - beta hcg within 12 days of the
IUI or a urine test 15 days after the IUI. So, you should know if you
are pregnant by now. if you are, consult your gynec. to give you
medication to try and stop the bleeding and have an ultrasound done at 6

fibroid in the uterus. If that is normal, have your thyroid hormones
checked. Regarding pregnancy, do an ultrasound on day 12 of the period
to ensure that your follicles are around 16 mm and you are ovulating.

Hi Doctor, I'm 25 years, unmarried girl m very
disturbed, actually 6 month before I went to the doctor because my
period are irregular then I got to know that I'm suffering form
bilateral enlarged Ovaries? Polycystic ovaries. And multiple small sized
follicles are seen bilaterally, no free fluid seen in pelvis. Volume of
Right overy is 5.8cc and Volume of left side Overy 8.5cc and my Uterus
is anteverted n anteflexed. And prolactin is also very high but know
it's in control and it's .77... no milk come out form my breast mam
please tell me that after marriage it'll create a problem in conceiving
or what should I do... m taking treatment for that.. for allabove
problems my taking dina 21 tablets and Cabogoline .25 please assist me.

Answer: PCO is present in 10- 20 % of women and is nothing to
worry or get frustrated about. You have to accept it as part of your
life. You would get irregular periods with that and due to the high
prolactin. if you do not wish to take all the medication now, only take
the carbegolin and see whether your periods are normal. Have your FSH &
LH done on day 2 of the period. If these are normal, you should
conceive. If you wish , you can take clomid for one cycle and monitor on
ultrasound whether you are ovulating or not. This would confirm that you
will not have any problems regarding conceiving. |

Answer: You must do a scan on day 11 of the cycle and determine
whether you are ovulating on the side that the tube is patent. Also,
what is your husband's semen result ? Have you done a PCT ( post- coital
test ) ? This would give some indication whether the sperms are dying in
the cervix and whether you need IUI. |

Answer: Did you not check with your gynec. before you did the
hysteroscopy? Anyway, the uterus is NOT pulled out of the vagina !
Saline is inserted through a hysteroscope into the uterus and the walls
of the uterus examined for any abnormalities. If there are any small
polyps or adhesions, they are removed. |

Since last year I was trying to conceive with my
husband and got pregnant, had miscarriage, had to do DNC. Before I got
married I had an abortion and then a miscarriage agian and had DNC as
well. It is being a a year trying with no success, tried one

Answer: Sorry to hear about your problems. Since you are 38 years
old, the chances of abortion are high due to a higher incidence of
chromosomal abnormalities. You must not waste any more time and try 2-3
cycles of IUI or one cycle of IVF , if finance permits. If not, you can
consider adoption. |

Hi doctor, this is the second time am writing to
you. please reply. am 24 yr old,single. I have a problem of vaginal
itching.
let me tell u the medications I have undergone:
i) initially I wld use coconut oil
ii) wash with salt water
iii) used candid B cream
iv) I was asked to use lactacyd lotion.
Sometimes still it itches.i use statum plus upon which i will be
relieved from itching for somedays. I have pale whitish/white discharge.
sometime very pale yellowish(bit pus color) which wont be much. during
medication i was asked to take Forcan tablet, metrogyl tablet during
which there was no itching. can u please help me to get rid of this. I
think as of discharge the area will be wet and itching starts. One more
thing: during periods very rarely it itches

Answer: You can use travocort or momenta cream locally twice a
day for 5 days on the vulva. Have a vaginal smear and culture and
sensitivity test done to determine the type of infection you have. this
will determine the most appropriate treatment. |

Hi! I am 27 years old married since four years and
have one daughter who is three years old . I have a history of one
abortion after my daughter, as I could not think of carrying the
pregnancy further because my daughter was too young that time. So I got
a surgical abortion done. Now that I want to conceive again it is not

months of natural try, my doctor gave me siphene tablets for five days
in the third month and susten 300 for the last 15 days. She also gave me
an injection so that the egg ruptures on time. My ovulation was positive
with 9.5cms. About still after trying for the third month also I could
not conceive, so now she wants to gave me placentrex injections
alternatively for 10 days. Ashe feels that I might have an inflammation
in the tubes or some blockage due to the abortion. please guide me
,whether I am following the right procedure or not and can this abortion
make me infertile? |

Answer: Placentrex might help reduce any inflammation in the tubes. Have you
done an HSG( hysterosalpingography)? This would determine if the tubes
are o.k.The follicle should be 18 or 20 mm. Please recheck the size of
the follicle. 9.5 cms would be a cyst ! Also, have your husband's semen
checked. let me know all the results, so I can advice you further. ALL

Answer: You can do a vaginal ultrasound during your periods.However, if you are
still bleeding, your gynec. would not be able to do a per speculum
examination adequately. That would reveal if you have a polyp or an
erosion on the cervix which can give rise to spotting. You might be
having heavy periods due to fibroids in the uterus. If the scan is
normal, have your thyroid hormones checked. |

Answer: Let your husband continue the medication for at least 2
months for adequate effect. What is his sperm motility and his serum FSH
? Also, are you ovulating ? What was the size of the follicle on the
14th day of the period ? Let me know all these, so I can advice you
further. |

Answer: A Hysterosalpingography is done as an out- patient
procedure and does not require any anaesthesia. So, it is safer than
laparoscopy. People CANNOT get infertile after that or laparoscopy. it
maybe that on doing these examinations, a blockage of the tubes maybe
found which is the reason for the infertility. |

Hello doctor, I am 27 year old and my husband is
32. I have been trying to conceive for last two years but in vain. I
have gone through follicle study atleast 4 months continuesly and had
intercourse accordingly but it did not help. there was no artifical
insemnation done. I have gone through numerous tests all have been okay.
Progestrone, semen analysis, sonography for various reasons and their is
no finding of any iregularity these days i am taking duphastine,
ultrafoli, progynova, and glyciphage to inhance the changes. Please
suggest if i should go for laproscopy or something else can be done.
Once artificial insemnation has also failed.

Answer: Sorry to hear about your problems. Have your tubes been
checked by hysterosalpingography? If not, you should have that done . It
is done as an out- patient procedure and does not require any
anaesthesia. If the tubes are normal, you can try IUI for 4-6 cycles. If
you still do not conceive then try IVF. All the best ! |
